It invents a plausible word and presents it with the same confidence as the rest. In a meeting note that's an annoyance. In a memoir it is a sentence your mother never said, sitting inside quotation marks, in a book her grandchildren will read as fact.
Read the transcript against the audio for anything that will be quoted. It takes minutes and it is the single highest-value hour in the whole process.
